Documentation Guidelines
Documentation must be provided by a qualified licensed professional who is familiar with the student’s condition and its functional limitations (e.g., physician, psychologist, psychiatrist, licensed counselor, or other appropriate healthcare professional).
Documentation is required on letterhead from a qualified professional stating diagnosis, functional limitations necessitating the accommodations and estimated length of time services will be needed.
An Individualized Education Program (IEP), Section 504 Plan, or Summary of Performance may be submitted as supporting documentation. However, additional documentation may be required if the information provided does not sufficiently describe the student’s current functional limitations and accommodation needs.
Documentation should include, when applicable:
- The name, credentials, licensure, and contact information of the evaluating professional.
- A diagnosis or description of the disability or medical condition, when appropriate.
- The date of the evaluation or most recent assessment.
- A description of the evaluation methods, procedures, or information used to reach the diagnosis or conclusion.
- A description of the current functional limitations resulting from the disability or condition.
- Information regarding the expected duration, progression, or stability of the condition, if known.
- Information about current or previously used accommodations, services, treatments, assistive technology, and/or medications, when relevant.
- Recommendations for accommodations or support services, including an explanation of how the recommendations relate to the student’s functional limitations.
The College reserves the right to request additional information when the documentation provided does not adequately support the requested accommodations or when clarification is needed to determine reasonable accommodations.
